体外冲击波联合下肢皮牵引治疗早中期膝骨关节炎42例临床观察（临床研究）

　　【摘　要】目的：观察下肢皮牵引联合体外冲击波治疗早中期膝骨关节炎的临床疗效。方法：将83例早中期膝骨关节患者按照随机数字表法分为治疗组42例和对照组41例。对照组给予下肢皮牵引治疗，治疗组在对照组基础上给予体外冲击波治疗。2组疗程均为4周。观察2组临床疗效，以及治疗前后日本骨科协会评分（JOA评分）。结果：治疗组临床治愈13例，显效23例，有效4例，无效2例，总有效率为95.23%；对照组临床治愈9例，显效15例，有效10例，无效7例，总有效率为82.92%。2组比较，差异有统计学意义（P < 0.05）。治疗结束时，2组JOA评分较治疗前均明显提高（P < 0.05），且治疗组优于对照组（P < 0.05）。结论：体外冲击波联合下肢皮牵引治疗早中期膝骨关节炎疗效显著，安全舒适痛苦小，值得临床推广。

Clinical Observation of 42 Cases of Knee Osteoarthritis in Early and Middle Stage Treated with Extracorporeal Shock Wave Combined with Lower Limb Skin Traction

　　【ABSTRACT】Objective:To observe the clinical effect of lower limb skin traction combined with extracorporeal shock wave in the treatment of knee osteoarthritis in early and middle stage.Methods:Eighty-three cases of knee joint patients in early and middle stage were randomly divided into a treatment group（42 cases）and a control group（41 cases）.The control group was given lower limb skin traction,and the treatment group was given extracorporeal shock wave therapy based on the treatment for the control group.Both groups were treated for 4 weeks.The clinical efficacy and JOA score before and after treatment were observed.Results:In the treatment group,13 cases were recovered,23 cases were markedly effective,4 cases were effective and 2 cases were ineffective,and the total effective rate was 95.23%.In the control group,9 cases were recovered,15 cases were markedly effective,10 cases were effective and 7 cases were ineffective,and the total effective rate was 82.92%.The difference between the two groups was statistically significant（P < 0.05）.At the end of treatment,JOA scores of both groups were significantly improved（P < 0.05）,and the treatment group was better than the control group（P < 0.05）.Conclusion:Extracorporeal shock wave combined with lower limb skin traction in the treatment of early and mid-term knee osteoarthritis has a significant effect,safe,comfortable and less pain,which is worthy of clinical promotion.
