温通止痛膏联合股四头肌功能锻炼治疗早期寒湿痹阻型膝骨关节炎45例临床观察（临床研究）

　　【摘　要】目的：观察温通止痛膏联合股四头肌功能锻炼治疗早期寒湿痹阻型膝骨关节炎的临床疗效。方法：将90例寒湿痹阻型膝骨关节炎患者随机分为治疗组和对照组，每组45例。治疗组予外敷温通止痛膏配合股四头肌功能锻炼，对照组予口服双氯芬酸钠缓释片。2组均以2周为1个疗程。观察2组临床疗效，以及治疗前后视觉模拟评分法（VAS）评分、Lequesne指数，同时观察2组治疗过程中不良反应。结果：治疗组临床控制10例，显效20例，有效8例，无效3例，总有效率为92.68%；对照组临床控制6例，显效18例，有效13例，无效4例，总有效率为90.02%。2组比较，差异无统计学意义（P > 0.05）。治疗后，2组VAS评分、Lequesne指数较治疗前均明显改善（P < 0.01）；且治疗组优于对照组（P < 0.05）。治疗期间，治疗组出现过敏反应3例，对照组出现消化道不良反应4例，2组不良反应发生率比较，差异无统计学意义（P > 0.05）。结论：对早期寒湿痹阻型膝骨关节炎采用外敷温通止痛膏联合股四头肌功能锻炼治疗，可有效缓解膝关节疼痛症状、改善膝关节功能，安全性良好。

Clinical Observation on Treating 45 Cases of Early Knee Osteoarthritis with Cold-dampness Obstruction Syndrome with Wentong Zhitong Gao（温通止痛膏）Combing Quadriceps Femoris Functional Exercise

　　【ABSTRACT】Objective:To observe the clinical effect of Wentong Zhitong Gao（温通止痛膏）combined with quadriceps femoris functional exercise in the treatment of early knee osteoarthritis with cold-dampness obstruction syndrome.Methods:Ninety cases of knee osteoarthritis were randomly divided into a treatment group and a control group,45 cases in each.The treatment group was externally given Wentong Zhitong Gao combined with quadriceps femoris functional exercise,while the control group was given diclofenac sodium sustained-release tablets.Two weeks was a course of treatment for both groups.The clinical efficacy,VAS and Lequesne index before and after treatment were observed,and the adverse reactions during treatment were also observed.Results:In the treatment group,10 cases were clinically controlled,20 cases were markedly effective,8 cases were effective,3 cases were ineffective,and the total effective rate was 92.68%;while in the control group,6 cases were clinically controlled,18 cases were markedly effective,13 cases were effective,4 cases were ineffective,and the total effective rate was 90.02%.There was no significant difference between the two groups（P > 0.05）.After treatment,VAS and Lequesne index of the two groups were significantly improved（P < 0.01）,and the treatment group was better than the control group（P < 0.05）.During the treatment,there were 3 cases of allergic reactions in the treatment group and 4 cases of gastrointestinal adverse reactions in the control group.There was no significant difference in the incidence of adverse reactions between the two groups（P > 0.05）.Conclusion:External application of Wentong Zhitong Gao combined with quadriceps femoris functional exercise can effectively relieve the symptoms of knee joint pain and improve the knee joint function with good safety.
