填髓胶囊联合聚焦式体外冲击波治疗早期肾虚血瘀型股骨头坏死40例临床观察（临床研究）

　　【摘　要】目的：观察填髓胶囊联合聚焦式体外冲击波对早期肾虚血瘀型股骨头坏死的临床疗效。方法：将80例肾虚血瘀型早期股骨头坏死患者随机分为治疗组和对照组，每组40例。治疗组采用中药填髓胶囊联合聚焦式体外冲击波治疗，对照组采用聚焦式体外冲击波治疗。观察2组优良率以及治疗前后疼痛视觉模拟评分法（VAS）评分、髋关节Harris评分，同时依照影像学量化指标进行ARCO分期变化比较。结果：治疗后6个月、12个月，2组VAS评分、Harris评分较治疗前均有改善（P < 0.05）；且治疗组优于对照组（P < 0.05）。治疗后12个月随访时，治疗组Harris优良率为80.00%，明显优于对照组的52.50%（P < 0.05）；治疗后ARCO分期比较，治疗组亦优于对照组（χ² = 5.49，P < 0.05）。结论：填髓胶囊联合聚焦式体外冲击波治疗早期肾虚血瘀型股骨头坏死临床疗效确切，优于单一聚焦式体外冲击波治疗。

Clinical Observation on Treating 40 Cases of Osteonecrosis of the Femoral Head of Kidney Deficiency and Blood Stasis Type with Tiansui Jiaonang（填髓胶囊）Combined with Focused Extracorporeal Shock Wave

　　【ABSTRACT】Objective:To observe the clinical effect of Tiansui Jiaonang（填髓胶囊）combined with focused extracorporeal shock wave on early osteonecrosis of the femoral head of kidney deficiency and blood stasis type.Methods:Eighty cases of early osteonecrosis of the femoral head of kidney deficiency and blood stasis type were randomly divided into a treatment group and a control group,40 cases in each.The treatment group was treated with traditional Chinese medicine Tiansui Jiaonang combined with focused extracorporeal shock wave,while the control group was treated with focused extracorporeal shock wave.The excellent and good rate,VAS score and Harris score of hip joint before and after treatment were observed.At the same time,ARCO staging changes were compared according to imaging quantitative indexes.Results:After 6 months and 12 months of treatment,VAS score and Harris score of both groups were improved（P < 0.05）,and the treatment group was better than the control group（P < 0.05）.The follow-up after 12 months' treatment showed that the Harris excellent and good rate in the treatment group was 80.00%,which was significantly better than 52.50% of the control group（P < 0.05）;Comparison of ARCO staging after treatment showed that the treatment group was also better than the control group（χ² = 5.49，P < 0.05）.Conclusion:The curative effect of Tiansui Jiaonang combined with focused extracorporeal shock wave in the treatment of early osteonecrosis of the femoral head of kidney deficiency and blood stasis type is better than that of focused extracorporeal shock wave only.
