RAPID3在类风湿关节炎患者中的临床应用价值探讨

　　【摘　要】目的：探讨患者数据指标常规评估值3（RAPID3）在反映类风湿关节炎患者疾病活动度方面的表现。方法：收集186例类风湿关节炎患者的临床资料进行横断面研究，其中包括RAPID3问卷、28个关节疾病活动指数（DAS28）、临床疾病活动指数（CDAI）和简化疾病活动指数（SDAI），采用Spearman相关分析、kappa检验对不同疾病活动度类风湿关节炎患者的RAPID3与DAS28、CDAI、SDAI的相关性和一致性进行检验。结果：RAPID3与常用的疾病活动度测量工具DAS28-ESR、DAS28-CRP、CDAI呈显著相关（r = 0.75，0.75，0.71，P < 0.01），与SDAI呈中度相关（r = 0.69，P < 0.01）；RAPID3与DAS28-ESR存在较好的一致性（Kappa = 0.716），与DAS28-CRP、SDAI、CDAI存在中等一致，Kappa值分别为0.68，0.54，0.56。结论：RAPID3与传统的临床测量工具有较强的相关性和一致性，是一种方便可靠的监测疾病活动度的测量工具。

Clinical Application of RAPID3 in Patients with Rheumatoid Arthritis

　　【ABSTRACT】Objective:To explore the performance of routine assessment of patient index data 3（RAPID3）in reflecting disease activity in patients with rheumatoid arthritis.Methods:Clinical data of 186 rheumatoid arthritis patients were collected for cross-sectional study,including RAPID3 questionnaire,disease activity score 28（DAS28）,clinical disease activity index（CDAI）and simplified disease activity index（SDAI）.The consistency between RAPID3 and DAS28,CDAI,SDAI in rheumatoid arthritis patients with different disease activities was performed by Spearman's correlations,kappa coefficients.Results:RAPID3 was significantly correlated with DAS28-ESR,DAS28-CRP and CDAI（r = 0.75,0.75,0.71,P < 0.01）,and moderately correlated with SDAI（r = 0.69,P < 0.01）；RAPID3 has a best consistency with DAS28-ESR（Kappa = 0.716）,followed by DAS28-CRP（Kappa = 0.68）and then SDAI,CDAI（Kappa = 0.54,0.56）.Conclusion:RAPID3 showed good correlation and consistency with traditional clinical measurement methods,and is a reliable and convenient tool for monitoring disease activities.
