中药熏洗联合塞来昔布治疗踝骨关节炎40例临床观察

　　【摘　要】目的：观察中药熏洗联合塞来昔布治疗踝骨关节炎的临床疗效。方法：将80例踝骨关节炎患者随机分为治疗组和对照组，每组40例。对照组口服塞来昔布，治疗组在对照组治疗基础上加用院内制剂下肢洗伤方熏洗。2组均以2周为1个疗程，治疗2个疗程。观察2组临床疗效和治疗前后视觉模拟评分法（VAS）评分、美国矫形外科足踝协会（AOFAS）踝-后足评分，以及不良反应发生情况。结果：治疗组优13例，良19例，中6例，差2例，优良率为80.00%，总有效率为95.00%；对照组优8例，良17例，中9例，差7例，优良率为62.50%，总有效率为85.00%。2组比较，差异有统计学意义（P < 0.05）。治疗后，2组患者VAS评分、AOFAS评分均明显改善（P < 0.05），且治疗组优于对照组（P < 0.05）。2组不良反应发生率比较，差异无统计学意义

（P > 0.05）。结论：中药熏洗联合塞来昔布治疗踝骨关节炎具有较好的疗效。

Clinical Observation on 40 Cases of Ankle Osteoarthritis with Chinese-medicine Fumigation and Washing Combined with Celecoxib

　　【ABSTRACT】Objective:To observe the clinical effect of fumigation and washing with traditional Chinese medicine combined with celecoxib in the treatment of ankle osteoarthritis.Methods:Eighty patients with ankle osteoarthritis were randomly divided into a control group and a treatment group,40 cases in each group.The control group was given celecoxib orally,while the treatment group was required to wash and fumigate the lower limbs with hospital-made preparation based on the treatment for the control group.Two groups were treated with 2 weeks as a course,2 courses of treatment.The clinical efficacy,VAS,AOFAS ankle-hind foot score and adverse reactions were observed before and after treatment.Results:In the treatment group,13 cases were excellent,19 cases were good,6 cases were at intermediate level,2 cases were not good.The excellent and good rate was 80.00%,and the total effective rate was 95.00%.In the control group,8 cases were excellent,17 cases were good,9 cases were at intermediate level,7 cases were not good.The excellent and good rate was 62.50%,and the total effective rate was 85.00%.There was significant difference between the two groups（P < 0.05）.After treatment,the VAS and AOFAS score of the two groups were significantly improved（P < 0.05）,and the treatment group was better than the control group（P < 0.05）.There was no significant difference in the incidence of adverse reactions between the two groups（P > 0.05）.Conclusion:Fumigation and washing with traditional Chinese medicine combined with celecoxib is effective in the treatment of ankle osteoarthritis.
