疏筋解结针刀闭合松解术对强直性脊柱炎患者功能活动的影响（临床研究）

　　【摘　要】目的：观察疏筋解结针刀闭合松解术对强直性脊柱炎患者日常活动能力的影响。方法：将60例强直性脊柱炎患者随机分为治疗组和对照组，每组30例。对照组给予常规药物口服治疗，治疗组在对照组治疗的基础上加用疏筋解结针刀闭合松解术，每周1次。2组均以4周为1个疗程。观察2组治疗前后患者临床症状、体征（包括脊柱疼痛评分、Schöber试验、晨僵时间和扩胸度）、Bath强直性脊柱炎疾病活动度（BASDAI）评分、强直性脊柱炎功能指数（BASFI）评分。结果：治疗后，2组在脊柱疼痛评分、Schöber试验、晨僵时间和扩胸度方面较治疗前均有改善，差异有统计学意义（P < 0.05）。治疗组晨僵时间、扩胸度、BASDAI评分、BASFI评分改善更为显著，与对照组比较，差异有统计学意义（P < 0.05）。结论：强直性脊柱炎患者在常规药物规范治疗基础上，通过疏筋解结针刀闭合松解术可进一步缓解患者疼痛症状，减少晨僵时间，显著增加脊柱关节的活动范围，具有疗程短、起效快等优点，值得临床推广应用。

Effect of Tendon-loosening Knife-needle Closure Release on Functional Activity of Patients with Ankylosing Spondylitis

　　【ABSTRACT】Objective:To observe the effect of tendon-loosening knife-needle closure release on the daily activities of patients with ankylosing spondylitis.Methods:Sixty patients with ankylosing spondylitis were randomly divided into a treatment group and a control group,30 cases in each group.The control group was routinely treated by taking drugs,while the treatment group was treated with tendon-relaxing knife-needle closure release once a week on the basis of the the treatment for the control group.Two groups were treated for four weeks as a course.The clinical symptoms and signs（including spinal pain score,Schöber test,morning stiffness time and chest enlargement）,bath ankylosing spondylitis disease activity index survey（BASDAI）and bath ankylosing spondylitis functional Index questionnaire（BASFI）were observed before and after treatment.Results:After treatment,the spinal pain score,Schöber test,morning stiffness time and chest enlargement in the two groups were improved compared with those before treatment,and the difference was statistically significant（P < 0.05）.In the evaluation of patients' daily activity ability,the morning stiffness time,chest enlargement,BASDAI and BASFI in the treatment group were improved more significantly than those in the control group,and the difference was statistically significant（P < 0.05）.Conclusion:On the basis of routine drug treatment,tendon-loosening knife-needle closure release can further relieve pain symptoms,reduce morning stiffness time,significantly increase the range of motion of the spine joint.It has the advantages of short course of treatment,quick onset,worthy of

clinical application.
