白细胞介素-22在类风湿关节炎中的临床意义（临床研究）

　　【摘　要】目的：探讨白细胞介素-22（IL-22）在类风湿关节炎中的临床意义。方法：选取类风湿关节炎患者49例（类风湿关节炎组），检测血浆IL-22水平、红细胞沉降率、C-反应蛋白、类风湿因子、抗环瓜氨酸肽抗体，记录患者关节压痛数、关节肿胀数，并计算疾病活动评分（DAS28）。分析IL-22与这些指标的相关性。健康对照组24例采用酶联免疫吸附法检测血浆IL-22。结果：①类风湿关节炎组血浆IL-22为（183.43±14.12）ng·L^-1，健康对照组为（105.33±11.12）ng·L^-1，2组比较，差异有统计学意义（P < 0.05）。②类风湿关节炎高活动组IL-22为（216.36±25.24）ng·L^-1，中活动组为（160.17±15.65）ng·L^-1，与低缓组（136.57±20.34）ng·L^-1比较，差异有统计学意义（P < 0.05）。③类风湿关节炎患者血浆IL-22水平与红细胞沉降率、关节压痛数、关节肿胀数、类风湿因子、C-反应蛋白及DAS28等指标呈正相关。结论：类风湿关节炎患者血浆IL-22较健康对照组升高，且与类风湿因子、红细胞沉降率、关节肿胀数及DAS28等活动性指标相关。IL-22可能参与了类风湿关节炎的发病过程。

Clinical Significance of Interleukin-22 in Rheumatoid Arthritis

　　【ABSTRACT】Objective:To investigate the clinical significance of interleukin-22（IL-22）in rheumatoid arthritis.Methods:Forty-nine patients with rheumatoid arthritis（the RA group）were selected.The level of IL-22 in plasma,erythrocyte sedimentation rate,C-reactive protein,rheumatoid factor and anti-CCP antibody were measured.The number of tenderness and swelling of joints were recorded and the disease activity score（DAS28）was calculated.The correlation between IL-22 and these indices was analyzed.The IL-22 in plasma was detected by enzyme-linked immunosorbent assay in 24 healthy controls.Results:①The IL-22 level in the RA group was（183.43±14.12）ng·L^-1,and that in healthy control group was（105.33±11.12）ng·L^-1.There was a significant difference between the two groups（P < 0.05）.②IL-22 was（216.36±25.24）ng·L^-1 in the RA high activity group and（160.17±15.65）ng·L^-1 in the RA medium activity group.There was a significant difference compared with the low activity group（136.57±20.34）ng·L^-1（P < 0.05）.③The level of IL-22 in plasma in RA patients were positively correlated with erythrocyte sedimentation rate,joint tenderness,joint swelling,rheumatoid factor,C-reactive protein and DAS28.Conclusion:The level of IL-22 in plasma of RA patients is higher than that in healthy controls,and it is related to rheumatoid factor,erythrocyte sedimentation rate,joint swelling and DAS28.IL-22 may be involved in the pathogenesis of RA.
