痛风急性期起始降尿酸治疗对临床疗效及用药依从性的影响（临床研究）

　　【摘　要】目的：探讨痛风急性期起始小剂量降尿酸治疗对临床疗效及患者用药依从性的影响。方法：将70例痛风急性期患者随机分为治疗组和对照组，每组35例。治疗组予抗炎治疗同时降尿酸治疗，对照组予急性期缓解2周后起始降尿酸治疗。治疗期间对疼痛视觉模拟评分法（VAS）评分进行比较，并随访尿酸水平、复发次数及用药依从性。结果：治疗后2组患者各时间点疼痛VAS评分比较，差异均无统计学意义（P > 0.05）。2组痛风复发比较，差异无统计学意义（P > 0.05）。治疗组尿酸较对照组水平更低，差异有统计学意义（P < 0.05）。2组用药依从性比较，差异有统计学意义（P < 0.05）。结论：痛风急性期起始降尿酸治疗不影响急性期疗效，可使尿酸更早达标且提高患者用药依从性。

Effect of Initial hypouric Acid Therapy for Acute Gout on Clinical Efficacy and Medication Compliance

　　【ABSTRACT】Objective:To investigate the effect of initial hypouric acid therapy for patients with acute gout on clinical efficacy and drug compliance.Methods:Seventy patients with acute gout were divided into an treatment group and a control group,35 cases in each group.The treatment group was treated with anti-inflammatory therapy and hypouric acid therapy and the control group with hypouric acid therapy after 2 weeks of acute remission.VAS pain scores were compared during treatment,and the levels of uric acid,recurrence times and medication compliance were followed up.Results:There was no significant difference in pain VAS scores between the two groups（P > 0.05）.There was no significant difference in gout recurrence between the two groups（P > 0.05）.The level of uric acid in the treatment group was lower than that in the control group（P < 0.05）.There was a significant difference in drug compliance（P < 0.05）.Conclusion:Initial hypouric acid therapy for acute gout does not affect the curative effect in the acute phase.It can make uric acid reach the standard earlier and improve the patient's medication compliance.
