五方散外敷联合双醋瑞因治疗膝骨关节炎30例临床观察

　　【摘　要】目的：观察五方散外敷联合双醋瑞因治疗膝骨关节炎的临床疗效。方法：将60例膝骨关节炎患者按随机数字表法分为治疗组和对照组，每组30例。对照组予口服双醋瑞因治疗，治疗组在对照组治疗基础上予五方散外敷膝关节，2组患者膝关节疼痛明显时可口服美洛昔康片治疗，同时记录药物消耗量。2组均以12周为1个疗程。观察2组疗效，以及治疗前后20 m步行痛、关节触痛和WOMAC评分，统计美洛昔康片的总用量。结果：治疗组有效28例，无效2例；对照组有效27例，无效3例。2组疗效整体评价比较，差异无统计学意义（P > 0.05）。治疗后，2组20 m步行痛、关节触痛和WOMAC评分较治疗前均有改善（P < 0.05），且治疗组优于对照组（P < 0.05）；治疗组美洛昔康的用量为（372.25±148.07） mg，对照组用量为（551.50±189.22） mg，2组比较，差异有统计学意义（P < 0.05）。结论：在双醋瑞因治疗的基础上加用五方散外敷，可明显减轻膝骨关节炎患者的关节疼痛，改善生活质量，减少抗炎镇痛药物美洛昔康用量。

Clinical Observation on 30 Cases of Knee Osteoarthritis Treated by External Application of Wufang San（五方散）Combined with Diacerein

　　【ABSTRACT】Objective:To observe the clinical efficacy of Wufang San（五方散）combined with diacerein in the treatment of knee osteoarthritis.Methods:Sixty patients with knee osteoarthritis were randomly divided into a treatment group and a control group,30 cases in each.The control group was treated with diacerein orally,and the knee joint of the treatment group was externally applied with Wufang San based on the treatment for the control group.Patients in both groups with obvious knee joint pain could take meloxicam tablets,and the drug consumption was recorded at the same time.Both groups were treated for 12 weeks as a course,observing their clinical efficacy,20 m walking pain,joint tenderness and WOMAC score before and after treatment,and the total dosage of meloxicam tablets was counted.Results:In the treatment group,28 cases were effective and 2 cases were ineffective;while 27 cases were effective and 3 cases were ineffective in the control group.There was no significant difference between the two groups（P > 0.05）.After treatment,the 20 m walking pain,joint tenderness and WOMAC score of the two groups were improved compared with those before treatment（P < 0.05）,and the treatment group was better than the control group（P < 0.05）.The dosage of meloxicam in the treatment group was（372.25±148.07）mg,and that of the control group was（551.50±189.22）mg,the difference being statistically significant（P < 0.05）.Conclusion:External application of Wufang San based on the treatment with diacerein can significantly reduce the joint pain of patients with knee osteoarthritis,improve their quality of life,and reduce the dosage of meloxicam.
