中医体质类型与类风湿关节炎相关性探析（临床研究）

　　【摘　要】目的：调查分析类风湿关节炎患者的中医体质分布，探求其与疾病的相关性。方法：根据《中医体质分类与判定自测表》对322例类风湿关节炎患者体质进行问卷调查分析。结果：①类风湿关节炎患者体质分布阳虚质最多，其次为瘀血质、气虚质、痰湿质；②中医证候以寒湿阻络证、痰瘀阻痹证居多，体质在不同证型中的分布差异有统计学意义（P < 0.05）；③痰湿质、湿热质男性多见，阳虚质、气虚质女性多见；④体质在不同年龄段、病程、关节功能分级及关节变形中的分布，差异有统计学意义（P < 0.05），其中病程 < 2年者阳虚质最多见，病程 > 5年者瘀血质、气虚质、痰湿质增多；关节功能Ⅲ级以上、有关节畸形者痰湿质、瘀血质、湿热质多见。结论：中医体质与类风湿关节炎有明显相关性，临床中应注意纠正患者偏颇体质，并根据体质特点合理调整用药，以提高临床疗效，改善预后。

Correlation Between TCM Constitution Types and Rheumatoid Arthritis

　　【ABSTRACT】Objective:To investigate and analyze the distribution of TCM constitution in patients with rheumatoid arthritis（RA）,and to explore its correlation with diseases.Methods:Three hundred and twenty-two patients with RA were investigated and analyzed by questionnaire according to Self-test Table for Classification and Judgment of TCM Constitution.Results:①The most distributed constitution was Yang deficiency,followed by blood stasis,qi deficiency and phlegm dampness;②TCM syndromes were mainly syndrome of cold dampness blocking collaterals and syndrome of phlegm stasis obstructing bi,and the distribution difference of constitution in different syndrome types was statistically significant（P < 0.05）;③Phlegm dampness and dampness heat constitutions were more common in male than in female mostly with yang deficiency and qi deficiency;④Age,course of disease,classification of joint function and joint deformation were different,and differences in constitution distribution had a statistical significance（P < 0.05）.Among them,those with less than two-year course of disease mostly had Yang deficiency constitution,and those with more than five-year course of disease increasingly had blood stasis,qi deficiency and phlegm dampness constitutions;those with joint function of gradeⅢ or above and with joint deformity mostly had and phlegm dampness,blood stasis and damp heat constitutions.Conclusion:There is an obvious correlation between TCM Constitution and RA.Attention should be paid to the correction of the biased constitution in clinical practice,adjusting medication reasonably according to the physical characteristics so as to improve the clinical efficacy and prognosis.
