加味乌头汤外洗联合西药对类风湿关节炎患者手关节炎症状改善的临床观察（临床研究）

　　【摘　要】目的：观察加味乌头汤外洗联合西药对类风湿关节炎患者手关节炎症状改善的临床疗效。方法：将60例类风湿关节炎患者随机分为治疗组和对照组，每组30例。对照组口服甲氨蝶呤及塞来昔布胶囊治疗，治疗组在对照组治疗基础上加用加味乌头汤外洗双手关节。2组均以60 d为1个疗程。观察2组临床疗效，以及治疗前后主要症状、体征和实验室指标变化情况。结果：治疗组ACR20、ACR50、ACR70均高于对照组，2组比较，差异有统计学意义（P < 0.05）。治疗后，2组关节压痛数、关节肿胀数、晨僵时间、视觉模拟评分法评分、C-反应蛋白、红细胞沉降率、双手握力较治疗前均有改善（P < 0.05），且治疗组优于对照组（P < 0.05）。结论：加味乌头汤外洗联合西药对类风湿关节炎患者手关节炎症状的改善有明显的疗效。

Clinical Observation on the Improvement of Hand Joint Inflammation in Patients with Rheumatoid Arthritis by External Washing of Modified Wutou Tang（乌头汤）and Western Medicine

　　【ABSTRACT】Objective:To observe the clinical effect of modified Wutou Tang（乌头汤）combined with western medicine on the improvement of hand joint inflammation in patients with rheumatoid arthritis.Methods:Sixty patients with rheumatoid arthritis were randomly divided into a treatment group and a control group,30 cases in each group.The control group was treated with methotrexate and celecoxib capsules,while the treatment group was treated by washing hand joints with modified Wutou Tang based on the treatment for the control group.Both groups were treated for 60 days as a course.The clinical efficacy,main symptoms,signs and laboratory indicators of the two groups were observed before and after treatment.Results:ACR20,ACR50 and ACR70 in the treatment group were higher than those in the control group,and there was significant difference between the two groups（P < 0.05）.After treatment,the number of joint tenderness,joint swelling,morning stiffness,visual analogue score,C-reactive protein,erythrocyte sedimentation rate and hand grip were improved in both groups（P < 0.05）,and the treatment group was better than the control group（P < 0.05）.Conclusion:Modified Wutou Tang combined with western medicine has an obvious effect on improving the symptoms of hand joint inflammation in patients with rheumatoid arthritis.
