舒筋接骨汤熏洗加外敷治疗肝肾亏虚型膝骨关节炎38例临床观察（临床研究）

　　【摘　要】目的：观察舒筋接骨汤熏洗加外敷治疗肝肾亏虚型膝骨关节炎的临床疗效。方法：将76例肝肾亏虚型膝骨关节炎患者随机分为治疗组和对照组，每组38例。治疗组给予舒筋接骨汤熏洗加药渣外敷治疗，对照组给予开水熏洗加双氯芬酸二乙胺软膏外涂治疗。2组均治疗2周。观察2组临床疗效，治疗前后西安大略和麦克马斯特评分（WOMAC）骨关节炎指数、关节压痛评分、肿胀评分，及关节滑液中肿瘤坏死因子-α（TNF-α）、基质金属蛋白酶-13（MMP-13）的变化。结果：治疗组显效21例，有效15例，无效2例，总有效率为94.74%；对照组显效12例，有效16例，无效10例，总有效率为73.68%。2组比较，差异有统计学意义（P < 0.05）。治疗后，2组WOMAC骨关节炎指数、关节压痛评分、关节肿胀评分、TNF-α、MMP-13较治疗前均有改善（P < 0.05），且治疗组优于对照组（P < 0.05）。结论：舒筋接骨汤治疗肝肾亏虚型膝骨关节炎临床疗效确切，可有效改善膝骨关节炎的症状，抑制关节炎症，减轻关节破坏。

Clinical Observation on 38 Cases of Liver-kidney-deficiency Knee Osteoarthritis Treated by Washing and External Application of Shujin Jiegu Tang（舒筋接骨汤）

　　【ABSTRACT】Objective:To observe the clinical efficacy of washing and external application with Shujin Jiegu Tang（舒筋接骨汤）in the treatment of liver-kidney-deficiency knee osteoarthritis.Methods:Seventy-six cases of liver-kidney-deficiency knee osteoarthritis were randomly divided into a treatment group and a control group,38 cases in each group.The treatment group was treated by washing and external application with Shujin Jiegu Tang,while the control group was treated by topical application with Fosalin Ointment after being washed with boiled water.The two groups were treated for 2 weeks.The clinical efficacy,as well as the changes of WOMAC osteoarthritis index,joint tenderness,swelling,tumor necrosis factor-α（TNF-α）and matrix metalloproteinase-13（MMP-13）were observed before and after treatment.Results:In the treatment group,21 cases were markedly effective,15 cases were effective,2 cases were ineffective,and the total effective rate was 94.74%;in the control group,12 cases were markedly effective,16 cases were effective,10 cases were ineffective,and the total effective rate was 73.68%.The difference between the two groups was statistically significant（P < 0.05）.After treatment,WOMAC arthritis index,joint tenderness score,joint swelling score,TNF-α,MMP-13 were improved in both groups than those before treatment（P < 0.05）,and the treatment group was superior to the control group（P < 0.05）.Conclusion:Shujin Jiegu Tang has a definite clinical effect on liver-kidney-deficiency knee osteoarthritis,which can effectively improve the symptoms of knee osteoarthritis,inhibit arthritis and reduce joint damage.
