放散式体外冲击波联合针刀治疗肩关节周围炎30例近期临床疗效观察（临床研究）

　　【摘　要】目的：观察放散式体外冲击波（rESWT）联合针刀治疗肩关节周围炎的临床疗效。方法：将90例肩关节周围炎患者随机分为rESWT组、针刀组、联合组，每组30例。rESWT组给予rESWT治疗，针刀组给予针刀治疗，联合组给予rESWT加针刀治疗。2组均以3周为1个疗程。观察3组临床疗效及治疗前后疼痛视觉模拟评分法（VAS）评分、上肢关节活动度（ROM）评分。结果：联合组治愈5例，显效17例，有效7例，无效1例，总有效率为96.66%；针刀组治愈2例，显效11例，有效8例，无效9例，总有效率为70.00%；rESWT组治愈0例，显效3例，有效15例，无效12例，总有效率为60.00%。联合组临床疗效优于针刀组和rESWT组（P < 0.05）。治疗后，3组VAS评分、ROM评分较治疗前均有显著改善（P < 0.05），且联合组优于rESWT组和针刀组（P < 0.05）。结论：rESWT联合针刀治疗肩关节周围炎疗效明确，值得临床研究及推广。

Short-term Clinical Efficacy Observation on 30 Cases of Periarthritis of Shoulder Treated with Radial Extracorporeal Shock Wave Therapy Combined with Acupotomy

　　【ABSTRACT】Objective:To observe the clinical efficacy of radial extracorporeal shock wave therapy（rESWT）combined with acupotomy in the treatment of periarthritis of shoulder.Methods:Ninety patients with periarthritis of shoulder were randomly divided into a rESWT group,an acupotomy group and a combined group,30 cases in each group.The rESWT group was given the rESWT treatment,the acupotomy group was treated with acupotomy,and the combined group was treated with both methods.Both groups were treated for three weeks.The clinical efficacy,visual analogue scale（VAS）and range of motion（ROM）of the upper limbs were observed before and after treatment.Results:In the combined group,5 cases were cured,17 cases were markedly effective,7 cases were effective,1 case was ineffective,and the total effective rate was 96.66%.In the acupotomy group,2 cases were cured,11 cases were markedly effective,8 cases were effective,9 cases were ineffective,and the total effective rate was 70.00%.In the rESWT group,no cases  were cured,3 cases were markedly effective,15 cases were effective,12 cases were ineffective,and the total effective rate was 60.00%.The clinical efficacy of the combined group was better than that of the acupotomy group and the rESWT group（P < 0.05）.After treatment,the VAS and ROM score of the three groups were significantly improved（P < 0.05）,and the combined group was superior to the rESWT group and the acupotomy group（P < 0.05）.Conclusion:rESWT combined with acupotomy is effective in the treatment of periarthritis of shoulder,worthy of clinical research and promotion.
