清痹汤联合金黄散外敷治疗急性痛风性关节炎临床观察

　　【摘　要】目的：观察清痹汤联合金黄散外敷治疗急性痛风性关节炎的临床疗效。方法：将66例急性痛风性关节炎患者随机分为治疗组和对照组，每组33例。对照组予美洛昔康、别嘌醇口服治疗，治疗组在对照组基础上内服清痹汤、外敷金黄散。观察2组临床疗效及治疗前后疼痛视觉模拟评分法（VAS）评分、尿酸、红细胞沉降率、C-反应蛋白等指标。结果：治疗组痊愈19例，好转12例，无效2例，总有效率为93.94%；对照组痊愈10例，好转14例，无效9例，总有效率为72.73%。2组比较，差异有统计学意义（P < 0.05）。治疗后，2组VAS评分、尿酸、红细胞沉降率、C-反应蛋白较治疗前均有改善

（P < 0.05）；且治疗组优于对照组（P < 0.05）。结论：清痹汤联合金黄散外敷可有效缓解急性痛风性关节炎患者的关节疼痛，也可降低患者炎症反应和血尿酸水平。

Clinical Observation on the Treatment of Acute Gouty Arthritis with Qingbi Tang（清痹汤）Combining External Application of Jinhuang San（金黄散）

　　【ABSTRACT】Objective:To observe the clinical efficacy of Qingbi Tang（清痹汤）combining external application of Jinhuang San（金黄散）in the treatment of acute gouty arthritis.Methods:Sixty six patients with acute gouty arthritis were randomly divided into a treatment group and a control group,33 cases in each group.The control group was treated with meloxicam and allopurinol,while the treatment group was treated with Qingbi Tang combining external application of Jinhuang San based on the treatment for the control group.The clinical efficacy,visual analogue scale（VAS）score,uric acid（UA）,erythrocyte sedimentation rate（ESR）and C-reactive protein（CRP）of the two groups were observed before and after treatment.Results:In the treatment group,19 cases were cured,12 cases were improved,2 cases were ineffective,and the total effective rate was 93.94%.In the control group,10 cases were cured,14 cases were improved,9 cases were ineffective,and the total effective rate was 72.73%.The difference between the two groups was statistically significant（P < 0.05）.After treatment,the VAS scores,UA,ESR and CRP in the two groups were improved（P < 0.05）,and the treatment group was better than the control group（P < 0.05）.Conclusion:Qingbi Tang combined with Jinhuang San can effectively relieve joint pain in patients with acute gouty arthritis,and also can reduce inflammatory reaction and the level of UA .
