Mulligan动态关节松动术联合功能锻炼治疗轻度肩周炎30例临床观察（临床研究）

　　【摘　要】目的：观察Mulligan动态关节松动术联合功能锻炼治疗轻度肩周炎的临床疗效。方法：将60例轻度肩周炎患者随机分为治疗组和对照组，每组30例。对照组给予肩关节功能锻炼（爬墙牵伸 + 肩关节水平内收牵伸 + 挽背内旋牵伸），治疗组在对照组治疗基础上进行Mulligan动态关节松动术。观察2组患者治疗前后视觉模拟评分法（VAS）评分、Constant-Murley肩关节评分、肩关节自主活动度（AROM）。结果：治疗后1，3个月，2组VAS评分、Constant-Murley肩关节评分、AROM较治疗前均有改善（P < 0.05），且治疗组优于对照组（P < 0.05）。治疗过程中，2组患者均未发生皮下出血、肌肉肿胀等不良反应。结论：应用Mulligan动态关节松动技术结合肩关节功能锻炼治疗轻度肩周炎可缓解肩关节疼痛，改善肩关节活动功能，具有显著的疗效，且安全性好，值得临床推广。

Clinical Observation on Mulligan Dynamic Joint Mobilization Combined with Functional Exercise for the Treatment of 30 Cases of Mild Shoulder Periarthritis

　　【ABSTRACT】Objective:To observe the clinical efficacy of Mulligan dynamic joint mobilization combined with functional exercise in the treatment of mild shoulder periarthritis.Methods:Sixty patients with mild shoulder periarthritis were randomly divided into a treatment group and a control group,with 30 cases in each group.The control group were given shoulder joint functional exercise（wall climbing traction plus shoulder joint horizontal adduction traction,and back rotation traction）,while the treatment group received Mulligan dynamic joint mobilization surgery on the basis of the control group.Observe the VAS score,Constant-Murley shoulder joint score,and active range of motion（AROM）of shoulder joint of the two groups before and after treatment.Results:After one and three months of treatment,the VAS score,Constant-Murley shoulder joint score,and AROM of shoulder joint in both groups improved compared to before treatment（P < 0.05）,and the treatment group was better than the control group（P < 0.05）.During the treatment process,neither group of patients experienced any adverse reaction such as subcutaneous bleeding or muscle swelling.Conclusion:The application of Mulligan dynamic joint mobilization technology combined with shoulder joint function exercise in the treatment of mild scapulohumeral periarthritis can alleviate shoulder pain and improve shoulder joint mobility,having significant therapeutic effects and good safety,which is worthy of clinical promotion.
