针刀镜治疗重度肩周炎的短期疗效观察（临床研究）

　　【摘　要】目的：观察针刀镜治疗重度肩周炎的短期疗效。方法：选取2018年7月至2021年6月上海中医药大学附属光华医院关节康复科收治住院的重度肩周炎患者150例，随机分为针刀组和针刀镜组，每组75例。针刀组采用小针刀治疗，针刀镜组采用针刀镜治疗。观察2组临床疗效，以及入院时、治疗后1 d、治疗后10 d视觉模拟评分法（VAS）评分、Constant-Murley肩关节评分；比较2组住院天数。结果：治疗10 d后，针刀镜组治愈5例，好转70例，未愈0例，总有效率为100.00%；针刀组治愈2例，好转70例，未愈3例，总有效率为96.00%。2组比较，差异有统计学意义（P < 0.05）。治疗后1 d、10 d，2组VAS评分均明显降低，Constant-Murley肩关节评分均明显提高（P < 0.01）；且针刀镜组均优于针刀组（P < 0.01）。针刀镜组平均住院天数较针刀组更短（P < 0.01）。结论：针刀镜治疗重度肩周炎短期疗效显著，住院时间短。

Observation on the Short-term Efficacy of Needle Knife Endoscopy in the Treatment of Severe Scapulohumeral Periarthritis

　　【ABSTRACT】Objective:To observe the short-term efficacy of needle knife endoscopy in the treatment of severe scapulohumeral periarthritis.Methods:One hundred and fifty patients with severe Scapulohumeral periarthritis admitted to the Joint Rehabilitation Department of Guanghua Hospital Affiliated to Shanghai University of Chinese Medicine from July 2018 to June 2021 were randomly divided into a needle knife group and a needle knife endoscopy group,with 75 cases in each group.The needle knife group was treated with needle knife ,while the needle knife endoscopy group was treated with needle knife endoscopy.The clinical efficacy,visual analogue scale（VAS）and Constant-Murley shoulder joint score were observed respectively at admission,one day after treatment and 10 days after treatment.Compare the length of stay between the two groups.Results:After 10 days of treatment,5 cases were cured and 70 cases were improved in the needle knife endoscopy group,with a total effective rate of 100.00%;in the needle knife group,2 cases were cured,70 cases improved,and 3 cases did not recover,with a total effective rate of 96.00%.The difference between the two groups was statistically significant（P < 0.05）.On the first and tenth day after treatment,VAS in both groups was significantly reduced,and Constant-Murley shoulder joint scores were significantly increased（P < 0.01）;and the needle knife endoscopy group was better than the needle knife group（P < 0.01）.The average hospitalization days in the needle knife endoscopy group were shorter than those in the needle knife group（P < 0.01）.Conclusion:Needle knife endoscopy has a significant short-term therapeutic effect on severe scapulohumeral periarthritis and a short hospital stay.
