电针联合中药外敷治疗老年痛风急性期患者的临床疗效及对血清炎性因子的影响（临床研究）

　　【摘　要】目的：观察电针联合中药外敷治疗老年痛风急性期患者的临床疗效及对血清炎性因子的影响。方法：将98例老年痛风急性期患者按照随机数字表法分为治疗组和对照组，每组49例。对照组采用电针治疗，治疗组采用电针联合中药外敷治疗。观察2组临床疗效，以及治疗前后中医证候积分、血尿酸（UA）水平、血清炎性因子［白细胞介素-1β（IL-1β）、白细胞介素-6（IL-6）、C反应蛋白（CRP）］水平，并观察安全性。结果：治疗组临床控制15例，显效21例，有效10例，无效3例，总有效率为93.88%；对照组临床控制11例，显效16例，有效12例，无效10例，总有效率为79.59%。2组比较，差异有统计学意义（P < 0.05）。治疗后，2组中医证候积分、UA、IL-1β、IL-6、CRP水平较治疗前均降低（P < 0.05），且治疗组均优于对照组（P < 0.05）；不良反应发生率治疗组为6.12%，对照组为2.04%，2组比较，差异无统计学意义（P > 0.05）。结论：电针联合中药外敷治疗老年痛风急性期效果较佳，可显著降低UA水平，明显减轻炎症反应，且安全性较高。

Clinical Efficacy of Electroacupuncture Combined with External Application of Traditional Chinese Medicine in the Treatment of Elderly Patients with Acute Gout and Its Impact on Serum Inflammatory Factors

　　【ABSTRACT】Objective:To observe the clinical efficacy of electroacupuncture combined with external application of Chinese medicine on elderly patients with acute gout and the influence of serum inflammatory factors.Methods:Ninety-eight elderly patients with acute gout were randomly divided into a treatment group and a control group,with 49 cases in each group.The control group was treated with electroacupuncture,while the treatment group was treated with electroacupuncture combined with external application of Chinese medicine.Observe the clinical efficacy of both groups,as well as the TCM syndrome scores,levels of UA and serum inflammatory factors（IL-1β,IL-6,CRP）before and after treatment and the safety.Results:In the treatment group,15 cases were clinically controlled,21 cases were significantly effective,10 cases were effective,and 3 cases were ineffective,with a total effective rate of 93.88%;in the control group,11 cases were clinically controlled,16 cases were significantly effective,12 cases were effective,and 10 cases were ineffective,with a total effective rate of 79.59%.The difference between the two groups was statistically significant（P < 0.05）.After treatment,TCM syndrome scores and the levels of UA,IL-1β,IL-6 and CRP of both groups decreased compared with before treatment（P < 0.05）,and the treatment group was better than the control group（P < 0.05）.The incidence of adverse reactions was 6.12% in the treatment group and 2.04% in the control group.There was no statistically significant difference between the two groups（P > 0.05）.Conclusion:The combination of electroacupuncture and external application of Chinese medicine has a better effect on the acute stage of elderly gout,which can significantly reduce UA levels,significantly reduce inflammatory reactions,and has high safety.
