加味猪苓汤治疗阴虚湿热型膝关节退变性滑膜炎的临床观察（临床研究）

　　【摘　要】目的：观察加味猪苓汤治疗阴虚湿热型膝关节退变性滑膜炎的临床疗效。方法：选取2018年6月至2020年6月在福建中医药大学附属宁德中医院骨一科门诊就诊的阴虚湿热型膝关节退变性滑膜炎患者66例，随机分为治疗组和对照组，每组33例。对照组患者给予塞来昔布胶囊口服及氟比洛芬巴布膏外贴，治疗组患者给予加味猪苓汤冲服。2组均以2周为1个疗程，共治疗2个疗程。观察2组患者治疗前及治疗2周、治疗4周关节肿胀度评分、疼痛视觉模拟评分法（VAS）评分、Lysholm膝关节功能评分、中医证候积分，并比较2组临床疗效。结果：治疗4周后，治疗组关节肿胀改善程度、疼痛VAS评分、Lysholm膝关节功能评分、中医证候积分显著优于对照组（P < 0.05）；治疗组总有效率为90.90%，对照组为72.73%，差异有统计学意义（P < 0.05）。结论：加味猪苓汤治疗阴虚湿热型膝关节退变性滑膜炎临床疗效显著，不仅能促进膝关节积液的吸收，缓解膝关节肿胀和疼痛，改善膝关节功能，还能改善中医证候，值得临床推广应用。

Clinical Observation of Jiawei Zhuling Tang（加味猪苓汤）in the Treatment of Knee Degenerative Synovitis of Yin Deficiency and Damp Heat Type

　　【ABSTRACT】Objective:To observe the clinical effect of Jiawei Zhuling Tang（加味猪苓汤）on kneedegenerative synovitis of yin deficiency and damp heat type.Methods:From June 2018 to June 2020,66 patients with knee degenerative synovitis of yin deficiency and damp heat type were selected from the First Outpatient Department of Osteology,Ningde Hospital of Traditional Chinese Medicine,affiliated to Fujian University of Traditional Chinese Medicine.All cases were randomly divided into an observation group and a control group,with 33 cases in each.Those in the control group were given celecoxib capsule orally and flurbiprofen cataplasm externally;while those in the observation group were given Jiawei Zhuling Tang.The two groups were treated for 2 weeks.The joint swelling score,VAS,Lysholm knee function score and TCM syndrome score of the two groups before treatment,2 weeks and 4 weeks after treatment were observed,and the clinical effects of the two groups were compared.Results:After 4 weeks of treatment,the improvement degree of joint swelling,VAS,Lysholm knee function score and TCM syndrome score of the observation group were significantly better than those of the control group（P < 0.05）.The total effective rate of the observation group was 90.90% and that of the control group was 72.73%.The difference was statistically significant（P < 0.05）.Conclusion:The clinical effect of Jiawei Zhuling Tang in the treatment of knee degenerative synovitis of yin deficiency and damp heat type is significant.It can not only promote the absorption of knee effusion,alleviate knee swelling and pain,improve knee function,but also improve TCM syndrome,worthy of clinical application.
