重骨颗粒治疗肝肾亏虚型膝骨关节炎30例临床观察（临床研究）

　　【摘　要】目的：观察重骨颗粒治疗肝肾亏虚型膝骨关节炎患者的临床疗效以及对患者生活质量的影响。方法：将60例肝肾亏虚型膝骨关节炎患者随机分为治疗组和对照组，每组30例。治疗组予以口服重骨颗粒治疗，对照组予以口服硫酸氨基葡萄糖胶囊治疗。2组均以12周为1个疗程。观察2组临床疗效，以及治疗前后视觉模拟评分法（VAS）评分、膝骨关节炎严重程度指数（Lequesne MG评分）、西安大略和麦克马斯特大学骨关节炎指数评分（WOMAC评分）、中医证候积分、红细胞沉降率（ESR）、C反应蛋白（CRP）、白细胞介素（IL）-1β、IL-6、肿瘤坏死因子-α（TNF-α）、生活质量量表（SF-36）积分。结果：治疗组临床治愈1例，显效18例，有效8例，无效3例，总有效率为90.00%；对照组临床治愈0例，显效12例，有效10例，无效8例，总有效率为73.33%。2组比较，差异有统计学意义（P < 0.05）。治疗后，治疗组中医证候（关节疼痛、腰膝酸软、倦怠乏力、潮热盗汗）积分、VAS评分、Lequesne MG评分、WOMAC评分均优于对照组（P < 0.05），治疗组在降低ESR、CRP、IL-1β、IL-6、TNF-α水平方面优于对照组（P < 0.05），且治疗组在一般健康状况、生理职能、躯体疼痛、精神健康等生活质量方面较对照组均有明显改善（P < 0.05）。结论：重骨颗粒可改善肝肾亏虚型膝骨关节炎患者关节症状，提高患者生活质量，值得临床推广及应用。

Clinical Observation of Chonggu Keli（重骨颗粒）in the Treatment of 30 Cases of Knee Osteoarthritis with Liver and Kidney Deficiency

　　【ABSTRACT】Objective:To observe the clinical efficacy of Chonggu Keli（重骨颗粒）in the treatment of knee osteoarthritis with liver and kidney deficiency and its effect on patients' life quality.Methods:Sixty patients with knee osteoarthritis of liver and kidney deficiency were randomly divided into a treatment group and a control group,30 cases in each group.The treatment group was treated with Chonggu Keli and the control group was treated with glucosamine sulfate for 12 weeks as a course of treatment.The clinical effects,as well as VAS score,Lequesne MG score,WOMAC score,TCM syndrome score,ESR,CRP,IL-1β,IL-6,TNF-α and SF-36 score before and after treatment were observed.Results:In the treatment group,1 case was recovered,18 cases were markedly effective,8 cases were effective and 3 cases were ineffective.The total effective rate was 90.00%;In the control group,no case was recovered,12 cases were markedly effective,10 cases were effective and 8 cases were ineffective.The total effective rate was 73.33%.There was a significant difference between the two groups（P < 0.05）.After treatment,the TCM syndrome score（joint pain,sore waist and knees,fatigue,hot flashes and night sweats）,VAS score,Lequesne MG score and WOMAC score in the treatment group were better than those in the control group（P < 0.05）.The treatment group was better than the control group（P < 0.05）in decreasing the levels of ESR,CRP,IL-1β,IL-6 and TNF- α,and the treatment group was significantly better than the control group in general health status,physiological function,physical pain and mental health（P < 0.05）.Conclusion:Chonggu Keli can treat and improve the joint symptoms of knee osteoarthritis with liver and kidney deficiency,and improve patients’ life quality.It is worthy of clinical promotion and application.
