化瘀通络方治疗类风湿关节炎瘀血阻络证34例临床观察

　　【摘　要】目的：观察化瘀通络方治疗类风湿关节炎瘀血阻络证的临床疗效。方法：将72例瘀血阻络证类风湿关节炎患者按照随机区组设计随机分为治疗组和对照组，每组36例。治疗组给予化瘀通络方联合甲氨蝶呤治疗，对照组给予雷公藤多苷片联合甲氨蝶呤治疗。2组均以12周为1个疗程。采用中医证候疗效评定标准及国际公认的DAS28标准进行疗效评定。结果：治疗组完成34例，对照组完成33例。治疗组中医证候疗效总有效率为97.06%，对照组为96.97%，2组比较，差异无统计学意义（P > 0.05）。治疗后，2组中医症状积分与治疗前比较，差异有统计学意义（P < 0.01）；2组间在同一时间点比较，差异无统计学意义（P > 0.05）。治疗组DAS28疗效总有效率为94.12%，对照组为96.97%，2组比较，差异无统计学意义（P > 0.05）。2组治疗后8周、12周DAS28评分较治疗前下降，治疗后12周较治疗后4周下降（P < 0.05或P < 0.01）；2组间在同一时间点比较，差异无统计学意义（P > 0.05）。结论：化瘀通络方治疗类风湿关节炎瘀血阻络证与雷公藤多苷片疗效相当。

Clinical Observation on 34 Cases of Rheumatoid Arthritis with Blood Stasis Syndrome Treated by Huayu Tongluo Decoction（化瘀通络方）

　　【ABSTRACT】Objective:To observe the clinical efficacy of Huayu Tongluo Decoction（化瘀通络方）in the treatment of rheumatoid arthritis with blood stasis syndrome.Methods:Seventy-two rheumatoid arthritis patients with blood stasis syndrome were randomly divided into a treatment group and a control group,36 cases in each group.The treatment group was treated with Huayu Tongluo Decoction Combined with methotrexate,and the control group was treated with Tripterygium Glycosides combined with methotrexate.Both groups were treated for 12 weeks as a course.The curative effect was evaluated by TCM Syndrome Evaluation Standard and internationally recognized DAS28 standard.Results:A total of 34 cases in the treatment group and 33 cases in the control group were completed.The total effective rate of TCM syndrome in the treatment group was 97.06%,and 96.97% in the control group,the difference was not statistically significant（P > 0.05）.After treatment,there was significant difference in TCM symptom scores between the two groups before and after treatment（P < 0.01）;there was no significant difference between the two groups at the same time point（P > 0.05）.The total effective rate of DAS28 in the treatment group was 94.12%,and 96.97% in the control group.There was no significant difference between the two groups（P > 0.05）.The DAS28 scores at 8 and 12 weeks after treatment in the two groups decreased compared with those before treatment,and those at 12 weeks after treatment decreased compared with those at 4 weeks after treatment（P < 0.05 or P < 0.01）;there was no significant difference between the two groups at the same time point（P > 0.05）.Conclusion:Huayu Tongluo decoction has the same curative effect as tripterygium glycosides tablets in treating rheumatoid arthritis with blood stasis syndrome.
