针刀结合关节松动术治疗肩关节周围炎25例临床观察

　　【摘　要】目的：观察针刀结合关节松动术治疗肩关节周围炎的临床疗效。方法：将75例肩关节周围炎患者随机分为针刀组、关节松动组、结合组，每组25例。针刀组给予针刀配合自我锻炼治疗，关节松动组给予关节松动术配合自我锻炼治疗，结合组给予针刀配合关节松动术治疗。3组均以3周为1个疗程。观察3组临床疗效，以及治疗前后疼痛视觉模拟评分法（VAS）评分、肩关节活动度（ROM）、Constant肩关节功能（CMS）评分。结果：治疗后，3组患者VAS评分、ROM、CMS评分较治疗前均有改善,差异有统计学意义（P < 0.05）；且结合组优于针刀组、关节松动组，差异有统计学意义（P < 0.05）。结论：针刀结合关节松动术治疗肩关节周围炎疗效明确，值得临床推广。

Clinical Observation on 25 Cases of Scapulohumeral Periarthritis Treated with Acupotomy Combined with Joint Mobilization

　　【ABSTRACT】Objective:To observe the clinical effect of acupotomy combined with joint mobilization in the treatment of scapulohumeral periarthritis.Methods:Seventy-five patients with scapulohumeral periarthritis were randomly divided into an acupotomy group,a joint loosening group and a combination group,with 25 cases in each.The acupotomy group was treated with acupotomy combined with self-exercise therapy,the joint mobilization group was treated with joint loosening combined with self-exercise,and the combination group was treated with acupotomy combined with joint loosening.The three groups were treated for 3 weeks as a course.The clinical efficacy,VAS,ROM and CMS were observed before and after treatment.Results:After treatment,VAS,ROM and CMS of the three groups were improved compared with those before treatment,the difference being statistically significant（P < 0.05）;and the combination group was better than the acupotomy group and the joint loosening group,the difference being statistically significant（P < 0.05）.Conclusion:Acupotomy combined with joint loosening is effective in the treatment of scapulohumeral periarthritis,worthy of clinical promotion.
