基于关联规则和聚类分析挖掘刘健教授治疗类风湿关节炎的用药规律（临床研究）

　　【摘　要】目的：应用关联规则和聚类分析对刘健教授治疗类风湿关节炎的处方用药进行分析，探讨其治疗本病的用药规律。方法：收集刘健教授治疗类风湿关节炎患者的4600首处方，应用Excel 2015建立数据库归纳数据，进行频数分析，使用SPSS 23.0、SPSS Modeler 14.2进行数据挖掘。结果：①健脾化湿药、清热解毒药、活血化瘀药、祛风除湿药4类为刘健教授常用药物，大部分药物使用频率高达50%。②关联规则得出置信度与关联度较高的药物组合：红花-桃仁，蒲公英-白花蛇舌草，陈皮-姜半夏等。③聚类分析得出，红花、桃仁、茯苓、陈皮、丹参、甘草、薏苡仁、蒲公英、威灵仙、山药、豨莶草、白花蛇舌草、泽泻，鸡血藤，麦芽、忍冬藤，杜仲、白术、黄芩，姜半夏等5组。结论：刘健教授治疗类风湿关节炎用药特点鲜明，强调从脾论治，并配伍活血化瘀、清热解毒、祛风除湿类中药，同调肝脾肾，补虚泻实，标本兼治。

On Professor LIU Jian’s Medication Rules in Treating Rheumatoid Arthritis Based on Association Rule and Cluster Analysis

　　【ABSTRACT】Objective:To analyze Professor LIU Jian's medication rules in the treatment of rheumatoid arthritis using association rules and cluster analysis.Methods:A total of 4600 prescriptions from Professor Liu Jian for rheumatoid arthritis were collected,and the database was established by Excel 2015 to summarize the data and analyze the frequency.SPSS 23.0 and SPSS Modeler 14.2 were used for data mining.Results:①Four categories of medicinals were commonly used by Professor LIU Jian（most of them with frequency up to 50%）,i.e.medicinals of invigorating spleen and removing dampness,medicinals of clearing away heat and toxin,medicinals of promoting blood circulation and removing blood stasis,medicinals of removing wind and dampness.②According to the association rules,the drug combinations with higher confidence and association were Honghua（Flos Carthami）and Taoren（Semen Persicae）,Pugongying（Herba Taraxaci）and Baihuasheshecao（Herba Hedyotis Diffusae）,Chenpi（Pericarpium Citri Reticulatae）-Jiangbanxia（Rhizoma Pinelliae Praeparatum）.③ Cluster analysis showed that there were five groups:Honghua（Flos Carthami）,Taoren（Semen Persicae）,Fuling（Poria）,Chenpi（Pericarpium Citri Reticulatae）,Danshen（Radix et Rhizoma Salviae Miltiorrhizae）,Gancao（Radix et Rhizoma Glycyrrhizae）,Yiyiren（Semen Coicis）,Pugongying（Herba Taraxaci）,Weilingxian（Radix et Rhizoma Clematidis）,Shanyao（Rhizoma Dioscoreae）,Xixiancao（Herba Siegesbeckiae）,Baihuasheshecao（Herba Hedyotis Diffusae）,Zexie（RhizomaAlismatis）;Jixueteng（Caulis Spatholobi）;Maiya（Fructus Hordei Germinatus）,Rendongteng（Caulis Lonicerae Japonicae）;Duzhong（Cortex Eucommiae）,Baizhu（Rhizoma Atractylodis Macrocephalae）,Huangqin（Radix Scutellariae）;Jiangbanxia（Rhizoma Pinelliae Praeparatum）.Conclusion:Professor Liu Jian’s treatment for rheumatoid arthritis has distinct characteristics,emphasizing treating the spleen,combining medicinals of promoting blood circulation and removing blood stasis,medicinals of clearing heat and detoxification,and medicinals of eliminating wind and dampness,regulating the liver,spleen and kidney,correcting deficiency and excess,and targeting the root causes and symptoms simultaneously.
