透刺法结合运动疗法及手法治疗寒湿凝滞型肩关节周围炎28例临床观察（临床研究）

　　【摘　要】目的：观察阳陵泉透阴陵泉穴、条口透承山穴配合肩关节的运动训练及手法治疗寒湿凝滞型肩关节周围炎的临床疗效。方法：将56例肩关节周围炎患者按就诊顺序随机分为治疗组和对照组，每组28例。治疗组采用透刺法（阳陵泉透阴陵泉、条口透承山）配合肩关节的运动训练及手法治疗，对照组采用普通针刺配合肩关节的运动训练和手法治疗。2组均以6 d为1个疗程，治疗3个疗程。观察2组临床疗效，以及治疗前后肩关节疼痛视觉模拟评分法（VAS）评分和关节功能活动量表评分。结果：治疗组痊愈5例，显效12例，好转9例，无效2例，总有效率为92.86%；对照组痊愈3例，显效7例，好转13例，无效5例，总有效率为82.14%。2组比较，差异有统计学意义（P < 0.01）。治疗后，2组VAS评分和关节功能活动量表评分较治疗前均有改善（P < 0.05或P < 0.01），且治疗组优于对照组（P < 0.05）。结论：透刺法结合肩关节的运动训练及手法治疗是寒湿凝滞型肩关节周围炎的有效治疗方法。

Clinical Observation on Treating 28 Cases of Cold-dampness-stagnation-type Scapulohumeral Periarthritis by Penetration Needling Combined with Kinesitherapy and Manipulation

　　【ABSTRACT】Objective:To observe the clinical effect of Yanglingquan（GB34）through Yinlingquan（SP9）and Tiaokou（ST38）through Chengshan Chengshan（BL57）combined with shoulder joint exercise and manipulation in the treatment of cold-dampness-stagnation-type scapulohumeral periarthritis.Methods:Fifty-six patients with scapulohumeral periarthritis were randomly divided into a treatment group and a control group,

28 cases in each.The treatment group was treated by penetration needling in combination with shoulder joint exercise and manipulation,while the control group was treated with general acupuncture in combination with shoulder joint exercise and manipulation.Both groups were treated for 3 courses,with 6 days as a course.The clinical effects of the two groups,as well as the visual analogue score（VAS）and joint function activity score before and after treatment were observed.Results:In the treatment group,5 cases were cured,12 cases were obviously effective,9 cases were improved,2 cases were ineffective,and the total effective rate was 92.86%;in the control group,3 cases were cured,7 cases were obviously effective,13 cases were improved,5 cases were ineffective,and the total effective rate was 82.14%.The difference between the two groups was statistically significant（P < 0.01）.After treatment,VAS score and joint function activity score of the two groups were improved（P < 0.05 or P < 0.01）,and the treatment group was better than the control group（P < 0.05）.Conclusion:Penetration needling combined with shoulder exercise and manipulation is an effective treatment for the cold-dampness-stagnation-type scapulohumeral periarthritis.
