强直性脊柱炎患者血清IL-23、IFN-γ的表达水平及临床意义

　　【摘　要】目的：探讨强直性脊柱炎（AS）患者血清白细胞介素-23（IL-23）、γ-干扰素（IFN-γ）的水平及临床意义。方法：运用Luminex液相芯片技术检测AS患者（AS组）53例、类风湿关节炎（RA）患者（RA组）30例、健康体检者（正常对照组）30例的血清IL-23、IFN-γ水平。分析AS患者血清细胞因子与C-反应蛋白（CRP）、红细胞沉降率（ESR）和疾病活动指数（ASDAS-CRP）的相关性，比较治疗前后AS患者血清IL-23、IFN-γ水平变化情况。结果：①AS患者血清IL-23、IFN-γ水平明显高于正常对照组，低于RA组，差异均有统计学意义（P < 0.05）；②AS患者血清IL-23与CRP、ASDAS-CRP呈直线正相关（r = 0.349，P = 0.010；r = 0.327，P = 0.017）；AS患者血清IFN-γ与CRP、ASDAS-CRP呈直线相关（r = 0.454，P = 0.001；r = 0.402，P = 0.003）；AS患者血清IL-23、IFN-γ与ESR无线性相关（P > 0.05）；③治疗后，AS患者血清IL-23、IFN-γ水平较治疗前降低，差异有统计学意义（P < 0.05）。结论：IL-23、IFN-γ与AS的发展密切相关。

Expression and Clinical Significance of Serum IL-23 and IFN-γ in Patients with Ankylosing Spondylitis

　　【ABSTRACT】Objective:To investigate the levels and clinical significance of serum IL-23 and IFN-γ in patients with ankylosing spondylitis.Methods:The levels of serum IL-23 and IFN-γ in 53 patients with ankylosing spondylitis（the AS group）,30 patients with rheumatoid arthritis（the RA group）and 30 healthy controls（the control group）were detected by Luminex Liquid Chip Technique.The correlation between serum cytokines and CRP,ESR and ASDAS-CRP was analyzed,and the changes of IL-23 and IFN-γ levels in AS patients before and after treatment were compared.Results:①The serum levels of IL-23 and IFN-γ in the AS group were significantly higher than those in the normal control group and lower than those in the RA group,and the difference was statistically significant（P < 0.05）.②For AS patients,the levels of IL-23 were positively correlated with CRP and ASDAS-CRP（r = 0.349,P = 0.010;r = 0.327,P = 0.017）,and IFN-γ was positively correlated with CRP and ASDAS-CRP.（r = 0.454,P = 0.001;r = 0.402,P = 0.003）;and IL-23,IFN-γ and ESR had no linear correlation（P > 0.05）.③After treatment,IL-23,IFN-γ levels in AS patients were lower than before treatment,and the difference was statistically significant（P < 0.05）.Conclusion:IL-23 and IFN-γ are closely related to the development of AS.
