玻璃酸钠联合复方倍他米松治疗膝骨关节炎44例临床观察

　　【摘　要】目的：观察玻璃酸钠联合复方倍他米松治疗膝骨关节炎的临床疗效。方法：将88例膝骨关节炎患者随机分为治疗组和对照组，每组44例。对照组给予玻璃酸钠关节腔内注射，治疗组在对照组治疗基础上加用复方倍他米松关节腔内注射。观察2组临床疗效，以及治疗前后视觉模拟评分法（VAS）评分、美国特种外科医院膝关节评分（HSS评分），治疗过程中药物不良反应情况。结果：治疗组显效14例，有效26例，无效4例，总有效率为90.91%；对照组显效10例，有效20例，无效14例，总有效率为68.18%。2组比较，差异有统计学意义（P < 0.05）。治疗7 d后，2组VAS评分较治疗前均有降低，且治疗组低于对照组（P < 0.05）。治疗5周后，2组HSS评分较治疗前均有升高，且治疗组高于对照组（P < 0.05）。2组患者治疗期间均未发生明显药物不良反应。结论：玻璃酸钠联合复方倍他米松关节腔内注射治疗膝骨关节炎疗效显著，可以有效提高患者膝关节功能，缓解疼痛程度，且安全性较高。

Clinical Observation on 44 Cases Treated with Sodium Hyaluronate Combined with Compound Betamethasone

　　【ABSTRACT】Objective:To observe the clinical efficacy of sodium hyaluronate combined with compound betamethasone in the treatment of knee osteoarthritis.Methods:Eighty-eight patients with knee osteoarthritis were randomly divided into a treatment group and a control group,44 cases in each group.The control group was given intra-articular injection of sodium hyaluronate,and the treatment group was given intra-articular injection of compound betamethasone on the basis of the treatment for the control group.The clinical efficacy,visual analogue scale（VAS）,Hospital for Special Surgery Knee Score（HSS）and adverse drug reactions（ADR）were observed before and after treatment.Results:In the treatment group,14 cases were markedly effective,26 cases were effective,4 cases were ineffective,and the total effective rate was 90.91%,while in the control group,10 cases were markedly effective,20 cases were effective,14 cases were ineffective and the total effective rate was 68.18%.The difference between the two groups was statistically significant（P < 0.05）.After 7 days of treatment,the VAS of the two groups was lower than that before treatment,and that of the treatment group was lower than the control group（P < 0.05）.After 5 weeks of treatment,the HSS of the two groups was higher than that before treatment,and that of the treatment group was higher than the control group（P < 0.05）.No obvious adverse reactions were observed in the two groups during treatment.Conclusion:Intra-articular injection of sodium hyaluronate combined with compound betamethasone is effective in the treatment of knee osteoarthritis,which can effectively improve knee joint function and relieve pain,safe.
