针刀松解联合臭氧消融术治疗腰椎间盘突出症临床观察

　　【摘　要】目的：观察针刀松解联合臭氧消融术治疗腰椎间盘突出症的临床疗效。方法：将72例腰椎间盘突出症患者随机分为治疗组和对照组，每组36例。对照组单纯使用针刀松解进行干预，并配合进行腰背肌康复锻炼；治疗组在对照组治疗基础上联合使用臭氧消融术干预。在治疗前及治疗3个月后对2组直腿抬高试验度数和日本骨科协会评估治疗分数（JOA评分）评定，并观察疗效。结果：治疗组治愈15例，有效20例，无效1例，总有效率为97.22%；对照组脱落1例，治愈7例，有效19例，无效9例，总有效率为72.22%。2组比较，差异有统计学意义（P < 0.05）。治疗后，2组直腿抬高试验情况和JOA评分较治疗前均有改善（P < 0.01），且治疗组优于对照组（P < 0.05）。结论：无论从疗效评定还是改善临床症状方面，针刀松解联合臭氧消融术在腰椎间盘突出症患者的干预上都有明显优势，值得临床推广。

Clinical Observation on Needle-knife Loosening Therapy Combined with Ozone Ablation in the Treatment of Lumbar Disc Herniation

　　【ABSTRACT】Objective:To observe the clinical efficacy of needle-knife loosening therapy combined with ozone ablation in the treatment of lumbar disc herniation.Methods:Seventy-two patients with lumbar disc herniation were randomly divided into a treatment group and a control group,36 cases in each group.The control group was treated with needle-knife loosening therapy combined with lumbar back muscle rehabilitation exercise,and the treatment group was treated with ozone ablation on the basis of the treatment for the control group.Before treatment and 3 months after that,the straight leg raising test and JOA score evaluation were done to the two groups to observe the curative effect.Results:In the treatment group,15 cases were healed,20 cases were effective,1 case was invalid,and the total effective rate was 97.22%.In the control group,7 cases were healed,19 cases were effective,9 cases were invalid,1 case was shedding,and the total effective rate was 72.22%.The difference between the two groups was statistically significant（P < 0.05）.After treatment,results of straight leg raising tests and JOA scores showed improvement compared with those before treatment（P < 0.01）,and the treatment group was better than the control group（P < 0.05）.Conclusion:Both needle-knife loosening therapy and ozone ablation have obvious advantages in the intervention of the patients with lumbar intervertebral disc herniation,regardless of the effect evaluation or the improvement of clinical symptoms.It is worthy of clinical promotion.
