腰椎退行性疾病疼痛患者相关功能障碍现状及影响因素调查分析（临床研究）

　　【摘　要】目的：探讨腰椎退行性疾病疼痛患者相关功能障碍现状及其影响因素，为干预方案的制定提供理论依据。方法：采用便利抽样法，选取2022年6月至2022年12月在成都大学附属医院骨科、中医科就诊的288例腰椎退行性疾病患者为研究对象。采用一般资料调查表、Oswestry功能障碍指数量表、一般自我效能感量表、医学应对方式问卷进行问卷调查，采用SPSS 24.0进行单因素分析、多元线性回归分析，采用AMOS 21.0进行中介效应检验。结果：腰椎退行性疾病疼痛患者相关功能障碍得分为（37.94±9.71）分，处于中等水平；年龄、疼痛程度、疼痛模式、一般自我效能感和应对方式是疼痛患者相关功能障碍的影响因素（t = 2.280，3.616，-2.969，-8.570，-5.631，2.746，4.816，P均 < 0.05）；应对方式在一般自我效能感与功能障碍之间起部分中介作用（中介效应为-0.282，占总效应的36.91%）。结论：医务人员应重视腰椎退行性疾病疼痛患者相关功能障碍情况，重点关注老年群体及疼痛程度较重、呈持续疼痛模式的患者，指导患者进行合理的功能锻炼，联合健康宣教提升患者一般自我效能感，引导患者积极应对疾病的发生，帮助其实现自主功能锻炼，提高生活质量。

Investigation and Analysis of Functional Impairment Status and Influencing Factors in Painful Patients with Lumbar Degenerative Disease

　　【ABSTRACT】Objective:To explore the current status and influencing factors of pain-related functional disorders in patients with lumbar degenerative disease to provide theoretical basis for the formulation of intervention plans.Methods:Convenience Sampling was used to select 288 patients with lumbar degenerative disease who visited the Orthopedics and TCM Departments of Chengdu University Affiliated Hospital from June 2022 to December 2022 as the study subjects.A questionnaire survey was conducted using the General Information Questionnaire,Oswestry Disability Index Scale,General Self Efficacy Scale,and Medical Coping Style Questionnaire.SPSS 24.0 was used for univariate analysis and multiple linear regression analysis,and AMOS 21.0 was used for mediation effect testing.Results:The pain related dysfunction score of patients with lumbar degenerative disease was（37.94±9.71）,which was at a moderate level;Age,degree of pain,pain patterns,general self-efficacy,and coping strategies are influencing factors for pain-related functional impairments in patients（t = 2.280,3.616,-2.969,-8.570,-5.631,2.746,4.816,P < 0.05）;coping styles partially mediate the relationship between general self-efficacy and functional impairment（mediation effect was -0.282,accounting for 36.91% of the total effect）.Conclusion:Medical staff should pay attention to the pain-related functional disorders in patients with lumbar degenerative disease,focus on the elderly population and patients with severe pain and persistent pain patterns,guide patients to engage in reasonable functional exercise,combine health education to improve patients' general self-efficacy,guide patients to actively respond to the occurrence of diseases,help them achieve autonomous functional exercise,and improve their quality of life.
