化湿降浊饮治疗脾虚湿阻型老年痛风45例临床观察（临床研究）

　　【摘　要】目的：观察化湿降浊饮治疗脾虚湿阻型老年痛风的临床疗效。方法：将90例脾虚湿阻型老年痛风患者随机分为治疗组和对照组，每组45例。对照组给予非布司他片口服，治疗组给予化湿降浊饮口服。2组1个疗程均为12周。观察2组临床疗效，以及治疗前后血尿酸水平、中医体质和证候积分的变化，并评价2组患者的安全性。结果：治疗组临床控制11例，显效17例，有效14例，无效3例，总有效率为93.33%；对照组临床控制6例，显效14例，有效15例，无效10例，总有效率为77.78%。2组比较，差异有统计学意义（P < 0.05）。治疗后，2组中医证候积分较治疗前均下降（P < 0.05），且治疗组优于对照组（P < 0.05）。2组血尿酸水平较治疗前均下降（P < 0.05），但治疗组与对照组比较，差异无统计学意义（P > 0.05）。老年痛风患者中医体质中痰湿质、湿热质和气虚质较多；治疗后，治疗组痰湿质、湿热质明显减少（P > 0.05），对照组无显著变化（P > 0.05）。结论：化湿降浊饮治疗脾虚湿阻型老年痛风能明显降低血尿酸水平，同时改善中医体质，疗效显著。

Clinical Observation on the Treatment of 45 Cases of Elderly Gout of Spleen Deficiency and Dampness Obstruction Type with Huashi Jiangzhuo Yin（化湿降浊饮）

　　【ABSTRACT】Objective:To observe the clinical efficacy of Huashi Jiangzhuo Yin（化湿降浊饮）in treating elderly gout of spleen deficiency and dampness obstruction type.Methods:Ninety patients with elderly gout of spleen deficiency and dampness obstruction type were randomly divided into a treatment group and a control group,with 45 cases in each group.The control group was given oral administration of Febuxostat Tablets,while the treatment group was given oral administration of Huashi Jiangzhuo Yin.Both groups had a treatment period of 12 weeks.The clinical efficacy of two groups,as well as the changes in blood uric acid levels,TCM constitution,and syndrome scores before and after treatment were observed,and the safety of the two groups of patients was evaluated.Results:In the treatment group,11 cases were clinically controlled,17 cases were significantly improved,14 cases were effective,and 3 cases were ineffective,with a total effective rate of 93.33%;in the control group,6 cases were clinically controlled,14 cases showed significant improvement,15 cases were effective,and 10 cases were ineffective,with a total effective rate of 77.78%.The difference between the two groups was statistically significant（P < 0.05）.After treatment,the TCM syndrome scores of both groups decreased compared to before treatment（P < 0.05）,and the treatment group was better than the control group（P < 0.05）.The blood uric acid levels in both groups decreased compared to before treatment（P < 0.05）,but there was no statistically significant difference between the treatment group and the control group（P > 0.05）.In the TCM constitution of elderly gout patients,phlegm dampness,damp heat,and qi deficiency are the most common.After treatment,the treatment group showed a significant decrease in phlegm dampness and damp heat,while the control group showed no significant changes after treatment.Conclusion:Huashi Jiangzhuo Yin can significantly improve TCM constitution and overall therapeutic effect in the treatment of elderly gout of spleen deficiency and dampness obstruction type,while reducing blood uric acid levels,with significant therapeutic effect.
