痛痹泰汤联合塞来昔布胶囊治疗急性痛风性关节炎60例临床观察（临床研究）

　　【摘　要】目的：观察痛痹泰汤联合塞来昔布胶囊治疗急性发作的湿热蕴结型痛风性关节炎的疗效。方法：将120例湿热蕴结型痛风性关节炎患者随机分为治疗组和对照组，每组60例。2组均予基础治疗，对照组予塞来昔布胶囊口服，治疗组在对照组的基础上加痛痹泰汤治疗。2组均以2周为1个疗程。观察2组临床疗效，以及治疗前后视觉模拟评分法（VAS）评分、关节肿胀程度评分、关节活动能力评分和实验室指标变化情况。结果：治疗组临床控制8例，显效35例，有效13例，无效4例，总有效率为93.33%；对照组临床控制4例，显效29例，有效14例，无效13例，总有效率为78.33%。2组比较，差异有统计学意义（P < 0.05）。治疗后，2组患者VAS评分、关节肿胀程度评分、关节活动能力评分，以及红细胞沉降率、C反应蛋白、血尿酸水平较治疗前均有改善（P < 0.05）；且治疗组优于对照组（P < 0.05）。治疗过程中，2组均无不良反应发生。结论：痛痹泰汤联合塞来昔布胶囊治疗湿热蕴结型痛风性关节炎可以明显改善患者临床症状，降低炎症指标及血尿酸，值得临床推广应用。

Clinical Observation on the Treatment of 60 Cases of Acute Gouty Arthritis with Tongbitai Tang（痛痹泰汤）Combined with Celecoxib Capsules

　　【ABSTRACT】Objective:To observe the therapeutic effect of Tongbitai Tang（痛痹泰汤）combined with celecoxib capsules on acute onset of gouty arthritis of dampness heat accumulation type.Methods:One hundred and twenty patients with gouty arthritis of dampness heat accumulation type were randomly divided into a treatment group and a control group,with 60 cases in each group.The control group was given oral administration of celecoxib capsules,while the treatment group was treated with Tongbitai Tang based on the treatment like that for the control group.Both groups were treated for 2 weeks as a course of treatment.Observe the clinical efficacy of both groups,as well as VAS,joint swelling degree scores,joint mobility scores,and changes in laboratory indicators before and after treatment.Results:In the treatment group,8 cases were cured,35 cases were significantly improved,13 cases were effective,and 4 cases were ineffective,with a total effective rate of 93.33%.In the control group,4 cases were cured,29 cases were significantly improved,14 cases were effective,and 13 cases were ineffective,with a total effective rate of 78.33%.The difference between the two groups was statistically significant（P < 0.05）.After treatment,the VAS,joint swelling degree score,joint mobility score,as well as red blood cell sedimentation rate,C-reactive protein,and blood uric acid levels of both groups improved compared to before treatment（P < 0.05）;and the treatment group was better than the control group（P < 0.05）.During the treatment,no adverse reactions occurred in both groups.Conclusion:The combination of Tongbitai Tang and celecoxib capsules in the treatment of gouty arthritis with dampness heat accumulation can significantly improve clinical symptoms and reduce inflammatory indicators and blood uric acid,worthy of promotion and application in clinical practice.
