三黄散外敷治疗合并上消化道出血的急性痛风性关节炎28例临床观察（临床研究）

　　【摘　要】目的：观察三黄散外敷治疗合并上消化道出血的急性痛风性关节炎的临床疗效。方法：将56例合并上消化道出血的急性痛风性关节炎患者按照随机数字表法分为治疗组和对照组，每组28例。对照组给予氟比洛芬凝胶贴膏贴敷治疗，治疗组予三黄散外敷治疗。2组均以1周为1个疗程。观察2组临床疗效，以及治疗前后关节疼痛数字评分法（NRS）评分、C反应蛋白和患者对疗效的整体评价。结果：治疗组有效25例，无效3例，总有效率为89.29%；对照组有效21例，无效7例，总有效率为75.00%。2组比较，差异有统计学意义（P < 0.05）。治疗后，2组关节疼痛NRS评分和C反应蛋白较治疗前均有改善（P < 0.05），且治疗组优于对照组（P < 0.05）。结论：中药三黄散外敷治疗急性痛风性关节炎安全有效。

Clinical Observation on 28 Cases of Acute Gouty Arthritis Complicated with Upper Gastrointestinal Bleeding Treated by External Application with Sanhuang San（三黄散）

　　【ABSTRACT】Objective:To observe the clinical effect of Sanhuang San（三黄散）on acute gouty arthritis

complicated with upper gastrointestinal bleeding.Methods:Fifty-six patients with acute gouty arthritis complicated with upper gastrointestinal bleeding were randomly divided into a treatment group and a control group,with 28 cases in each group.The control group was treated with flurbiprofen gel plaster,while the treatment group was treated with Sanhuang San.One week was a course of treatment for both groups.The joint pain NRS scores before and after treatment,CRP and the overall evaluation of the efficacy,as well as the clinical efficacy of both groups were observed.Results:In the treatment group,25 cases were effective and 3 cases were ineffective.The total effective rate was 89.29%;In the control group,21 cases were effective and 7 cases were ineffective.The total effective rate was 75.00%.The difference between the two groups was statistically significant（P < 0.05）.After treatment,the joint pain NRS score and CRP of the two groups were improved compared with those before treatment（P < 0.05）,and the treatment group was better than the control group（P < 0.05）.Conclusion:Sanhuang San is safe and effective in the treatment of acute gouty arthritis.
