中医健康指导联合恒温雷火灸治疗强直性脊柱炎30例临床观察（临床研究）

　　【摘　要】目的：观察中医健康指导联合恒温雷火灸在强直性脊柱炎治疗中的临床价值。方法：将60例强直性脊柱炎患者采用随机数字表法分为治疗组和对照组，每组30例。除常规治疗及健康宣教外，对照组予传统雷火灸；治疗组予中医健康指导联合恒温雷火灸。观察2组临床疗效，以及治疗前后Bath强直性脊柱炎病情活动指数（BASDAI评分）、Bath强直性脊柱炎功能指数（BASFI评分）、疼痛视觉模拟评分法（VAS）评分、生活质量量表（SF-36）评分、护理满意度。结果：治疗组显效24例，有效5例，无效1例，总有效率为96.67%；对照组显效14例，有效7例，无效9例，总有效率为70.00%。2组比较，差异有统计学意义（P < 0.05）。治疗后，2组BASDAI评分、BASFI评分、VAS评分、SF-36评分较治疗前均有改善（P < 0.01）；且治疗组优于对照组（P < 0.05或P < 0.01）。治疗组护理满意度高于对照组，2组比较，差异有统计学意义（P < 0.05）。结论：中医健康指导联合恒温雷火灸治疗强直性脊柱炎，可提高患者生活质量及护理满意度，临床上应进一步推广应用。

Clinical Observation of 30 Cases of Ankylosing Spondylitis Treated by TCM Health Guidance Combined with Thunder-fire Moxibustion in Constant Temperature

　　【ABSTRACT】Objective:To observe the clinical value of TCM health guidance combined with thunder-fire moxibustion in constant temperature in the treatment of ankylosing spondylitis.Methods:Sixty patients with ankylosing spondylitis were randomly divided into a treatment group and a control group,30 cases in each group.In addition to routine treatment and health education,the control group was given traditional thunder-fire moxibustion;The treatment group was given TCM health guidance combined with thunder-fire moxibustion in constant temperature.The clinical efficacy,BASDAI score,BASFI score,VAS,SF-36 score and nursing satisfaction were observed before and after treatment.Results:In the treatment group,24 cases were markedly effective,5 cases were effective,1 case was ineffective,and the total effective rate was 96.67%;In the control group,14 cases were markedly effective,7 cases were effective,and 9 cases were ineffective,and the total effective rate was 70.00%.The difference between the two groups was statistically significant（P < 0.05）.After treatment,BASDAI score,BASFI score,VAS and SF-36 were improved in both groups（P < 0.01）,and the treatment group was better than the control group（P < 0.05 or P < 0.01）.The nursing satisfaction of the treatment group was higher than that of the control group,and the difference was statistically significant（P < 0.05）.Conclusion:TCM health guidance combined with thunder-fire moxibustion in constant temperature in the treatment of patients with ankylosing spondylitis can improve their quality of life and nursing satisfaction,which should be further promoted and applied in clinic.
